LumaDent is conducting a limited-time raffle to support the beta testing of our new IPD Checker Tool. Your participation helps us improve accuracy for future customers—and gives you a chance to win a $100 Amazon Gift Card.

By participating, you agree to the following terms:

1. Eligibility

To be eligible for the raffle, participants must:

• Complete IPD Checker Tool
  
• Enter their full name on the final slide
  
• Have a valid Interpupillary Distance (IPD) on file in LumaDent’s system
  

A valid IPD on file means:

• You previously purchased loupes through LumaDent and were professionally measured
  OR
• You were measured by a LumaDent Sales Representative using our in-person IPD process
  OR
• You are not yet a LumaDent customer but submit a valid optometrist-measured IPD using this form:
  Submit IPD and Enter Raffle
  (No Purchase Necessary)

This ensures we can compare your actual IPD to the tool’s results to validate accuracy.

2. No Purchase Necessary

This raffle is free to enter. You are not required to make a purchase to participate or win.

If you are not a LumaDent customer and do not already have an IPD on file, you may still enter the raffle by submitting your optometrist-measured IPD through the official form linked above.

3. Prize

• One (1) winner will be randomly selected to receive a $100 Amazon Gift Card.
• Winner will be notified via email after the beta testing period ends.
• Prize is non-transferable and may not be exchanged for cash.

4. Entry Limit

Each participant is eligible for one (1) raffle entry total, regardless of how many times they use the IPD Checker tool or submit measurements. Multiple submissions will not result in additional entries.

5. Entry Deadline

All entries must be submitted by September 12, 2025 to be eligible for the raffle.

6. General Conditions

• LumaDent reserves the right to disqualify any entrant who tampers with the entry process or does not follow instructions.
• By entering, you agree to let LumaDent use anonymized data from your IPD Checker result for research and product improvement purposes.
